What is Gynecomastia? The Difference Between True Gynecomastia and Pseudogynecomastia
Gynecomastia is the enlargement of glandular (breast) tissue, fatty tissue, or both in the male breast, leading to a feminine appearance. Correctly understanding the cause and type of this condition is the first and most important step in planning the treatment.
1. True Gynecomastia (Glandular Type)
In this type, the primary cause of breast enlargement is the growth of the gland tissue itself. There is typically a firm, disc-shaped mass palpable behind the nipple. This tissue, unlike fatty tissue, cannot be eliminated through sports or diet. Its only solution is surgical removal.
2. Pseudogynecomastia (Lipomastia)
In this type, the cause of enlargement is not an increase in breast gland tissue, but simply an excessive accumulation of fat in the area. It is directly related to weight gain. The breast tissue is generally soft, and no firm glandular mass is felt.
3. Mixed Type
This is the most common type of gynecomastia seen in men. The cause of enlargement is both the growth of the breast gland and an increase in the surrounding fatty tissue. The best and most aesthetic result is achieved by addressing both tissues simultaneously.
What Causes Gynecomastia?
Male breast enlargement usually stems from an imbalance between estrogen (female hormone) and testosterone (male hormone) in the body. This imbalance can arise from many different causes:
-  Physiological Gynecomastia: -  Newborn: Can occur temporarily due to the mother's estrogen. 
-  Adolescence: During this period of intense hormonal fluctuations, transient gynecomastia can be seen in more than half of males. In most cases, it resolves on its own within 1-2 years, but in some, it can become permanent.

Method 1: Gynecomastia Treatment with Liposuction
-  Who it's for: Applied only to patients with Pseudogynecomastia, meaning those without hard glandular tissue, where the enlargement is caused entirely by fat. 
-  How it's done: Through 1-2 very small (3-4 mm) incisions made at the edge of the breast or in the armpit, advanced technologies like Vaser® Liposuction are used to liquefy and suction out the fat tissue. 
-  Advantage: It is virtually scarless, and recovery is very fast. Technologies like Vaser also help in skin tightening. 
Method 2: Surgical Excision (Subcutaneous Mastectomy)
-  Who it's for: Applied to patients with only True Gynecomastia (Glandular Type), where there is little fat but a firm, distinct gland tissue behind the nipple. 
-  How it's done: A small semicircular incision is made over the lower half of the areola (the dark area around the nipple). The hard gland tissue is removed as a whole through this incision. The scar is hidden in the color transition zone, becoming almost imperceptible over time. 
Method 3: Combined Technique (Liposuction + Surgical Excision)
-  Who it's for: Applied to the vast majority of patients who have Mixed Type gynecomastia. 
-  How it's done: This is the gold standard method for the best aesthetic result. First, liposuction is used to remove excess fat from the breast and the sides of the chest, slimming and contouring the area. Then, through the same small incision at the areola, the hard gland tissue, which cannot be removed by liposuction, is surgically excised.

Gynecomastia Surgery Recovery Process
-  First Week: The operation is performed under local or general anesthesia, depending on the technique chosen, and lasts 1-1.5 hours. You can usually be discharged the same day. Pain is minimal for the first few days and is managed with medication. 
-  The Most Important Part: Gynecomastia Compression Vest: The gynecomastia compression vest is the most important part of the post-operative period. You will be fitted with a special medical vest immediately after surgery. This vest is vital for ensuring the skin adheres to the new underlying contour, reducing swelling, preventing bleeding, and achieving the best aesthetic result. 
-  Return to Normal Life: Most patients can return to desk work and daily life within 3-5 days after surgery.

Frequently Asked Questions About Gynecomastia Surgery
Will there be scars after gynecomastia surgery?
This depends on the technique chosen. If only liposuction is performed, only dot-like scars (3-4 mm) that fade over time remain. If surgical excision (gland removal) is required, the scar is hidden on the border between the dark areola and the normal skin tone. This scar becomes almost imperceptible over time due to the color transition. Except in rare cases of very advanced skin sagging, there are no significant visible scars.
Is gynecomastia surgery painful?
No. Gynecomastia surgery is known as a procedure where patients generally do not complain of pain. The sensation felt post-surgery is often described as "a feeling of pressure or tightness rather than pain," and this is very easily managed with simple pain relievers.
How long must the gynecomastia vest be worn?
This is the most critical part of recovery. The vest is generally required to be worn for 6 to 8 weeks as directed by your doctor (continuously for the first few weeks, then intermittently) to ensure the skin adheres to the new flat contour and to help swelling subside.
Can gynecomastia surgery be performed during adolescence?
If gynecomastia appears at the beginning of puberty, it is generally recommended to wait until age 19-20 for hormonal stabilization, as it may resolve on its own. However, in cases where it persists, becomes permanent, and severely affects the teen's psychological state (e.g., social isolation, avoiding school), surgery can be planned at ages 16-17.
Are the results of gynecomastia surgery permanent? Will it recur?
The results of gynecomastia surgery are permanent. The fat cells and gland (breast tissue) removed during the operation do not grow back. However, if the patient gains excessive weight (15-20 kg or more) after surgery, the remaining fat cells can expand, creating some fat accumulation in the area again. Maintaining an ideal weight is important for the permanence of the results.
Can gynecomastia be treated with sports or medication?
Sports and diet can only slightly reduce "pseudogynecomastia" caused solely by fatty tissue. However, if the cause of the enlargement is the hard "gland" (breast tissue), it is physiologically impossible for this tissue to be eliminated through sports or diet. The only permanent solution for this tissue is surgical removal.
