Breast Lift Surgery (Mastopexy): Regain Ideal Form

Breasts are essential complements to a feminine silhouette. However, unavoidable factors like time, gravity, pregnancy, breastfeeding, and significant weight fluctuations can lead to the most common concern: "sagging" (ptosis). Once full and firm breast tissue loses volume, appears deflated, the skin loosens, and the nipple points downwards. This situation becomes a problem directly affecting a woman's body image, self-confidence, and even clothing choices. It is at this point that Breast Lift Surgery (Mastopexy), one of the most artistic solutions offered by modern aesthetic surgery, comes into play.
The purpose of this operation is to reshape the sagging breast tissue and excess skin, reposition the nipple to its ideal location, and restore a youthful, lifted, and aesthetic form to the breasts. At his clinic in Istanbul, Assoc. Prof. Dr. Yalçın Bayram plans the most advanced surgical techniques with a personalized approach, aiming to provide you not just with lifted breasts but also with a natural and proportional appearance that helps you reconcile with your body.

What is Breast Lift Surgery (Mastopexy)?
Breast Lift Surgery is a "lifting" and "reshaping" procedure. Its difference from breast reduction surgery is that the goal here is not significantly reducing breast volume; the primary objective is correcting the position. The operation involves three fundamental steps:
-
Removal of Excess Skin: Loosened and stretched skin causing the sagging is surgically removed.
-
Reshaping of Breast Tissue: The underlying lax breast tissue (gland) is gathered and reshaped into a higher, firmer position.
-
Nipple Repositioning: The downward-pointing nipple-areola complex is moved upwards to its more youthful and ideal position, looking straight ahead. If necessary, the diameter of an enlarged areola is also reduced.
Causes of Sagging and Who is an Ideal Candidate for Breast Lift Surgery?
Breast sagging (ptosis) occurs as a result of the stretching of the ligaments supporting the breast (Cooper's ligaments) and the loss of skin elasticity. Ideal candidates for this operation are women experiencing aesthetic and psychological issues caused by breast sagging. Main Causes of Sagging:
-
Pregnancy and Breastfeeding: The most common cause. Breasts enlarge during pregnancy and fill with milk, then lose volume after breastfeeding ends. The skin often cannot retract sufficiently to accommodate this new smaller volume, leading to sagging (an "empty breast" appearance).
-
Gravity and Aging: The constant effect of gravity over the years and the decrease in skin elasticity with age make sagging inevitable.
-
Massive Weight Loss: Significant weight loss, especially after Post Bariatric Surgery, causes dramatic skin sagging.
-
Genetic Factors: Some women genetically have weaker skin structure or connective tissue, which can lead to early sagging. If your nipple is at or below the level of your inframammary fold (the crease under the breast), you may be a good candidate for breast lift surgery.
The Most Critical Decision: Lift Only, or Lift with Augmentation?
This is the most important decision for patients considering Mastopexy surgery. Often, the underlying cause of sagging is not just excess skin but also a loss of breast volume.
Breast Lift Surgery Only (Mastopexy Alone)
If the patient has sufficient breast volume (fullness) but experiences only sagging and excess skin, this method is applied. Excess skin is removed, and the existing breast tissue is reshaped and lifted.
Breast Lift with Augmentation Surgery (Augmentation-Mastopexy)
This is the most frequently preferred combination, especially after childbirth and breastfeeding, when breasts are both saggy and "deflated."
-
Why is it Necessary?: Performing only a lift elevates the breast but doesn't correct the deflated and empty appearance, particularly in the upper pole (cleavage area). It can result in breasts that look "lifted" but still "empty."
-
How is it Done?: During the same operation, Assoc. Prof. Dr. Yalçın Bayram performs the lifting procedure by removing excess skin and simultaneously places a breast implant under the breast tissue or muscle.
-
Result: Thanks to this combination, the breasts are both raised to their ideal position and regain their lost fullness (especially upper pole fullness). The outcome is a much younger, fuller, and more aesthetic appearance.
Breast Lift Surgery Techniques and Scars: What to Expect?
Breast lift surgery is an operation that leaves scars. The decision on which technique to use, and consequently the type of scar, depends on the degree of breast sagging.
1. Lollipop Scar Breast Lift (Vertical Mastopexy)
This is the most commonly preferred technique for moderate sagging.
-
Scar Location: The incision is made around the areola and vertically down from the areola to the inframammary fold (Lollipop shape).
-
Advantage: It does not leave a horizontal scar under the breast. It provides excellent projection (forward prominence) and shape to the breast.
2. Inverted-T Scar Breast Lift (Anchor Scar / Wise Pattern)
Used in cases with more advanced sagging (where the nipple is well below the inframammary fold) or when combined with Breast Reduction.
-
Scar Location: The incision includes the Lollipop scar plus an additional horizontal scar hidden in the inframammary fold (Inverted-T or Anchor shape).
-
Advantage: Provides the surgeon with complete access to the tissue, allowing for the removal of the maximum amount of sagging skin. This technique achieves the most dramatic and effective lift.
3. Around the Nipple Only (Periareolar / Donut Lift)
Preferred for very mild sagging or primarily for reducing the diameter of a very large areola. The scar is hidden only around the areola (like a donut). Its lifting power is much more limited compared to other techniques.
Can Non-Surgical Breast Lifts Be an Alternative to Surgery?
Patients often seek "non-surgical" solutions. Although methods like laser, radiofrequency, thread lifts, or "vampire breast lift" are marketed, none can replace a surgical mastopexy. These methods might provide minimal skin tightening but cannot repair stretched ligaments, remove excess skin, or lift the breast tissue. The only permanent and real solution for significant breast sagging is breast lift surgery.
Breast Lift Surgery Recovery Process
-
First Week: The operation is performed under general anesthesia and lasts 2-3 hours (longer if implants are added). Usually, a 1-night hospital stay is sufficient. Swelling, bruising, and tenderness in the area are normal. Drains may be placed.
-
2-4 Weeks: You will need to wear a special medical bra. Most patients can return to desk work within 7-10 days. Sutures are removed (if not dissolvable).
-
6-8 Weeks: Use of the medical bra continues during this period (especially at night). With your doctor's approval, you can begin light exercises and walks that do not strain your arms. Heavy lifting and strenuous sports should be avoided.
-
3-6 Months: It may take this long for the breasts to fully "settle," all swelling to disappear, and the breasts to achieve their final shape and softness. Scars are still typically red and raised during this period.
-
1-2 Years: It can take this long for the scars to mature, fade in color, and reach their final appearance.
Breast Lift Surgery Costs 2025
Breast lift surgery costs vary based on the extent of the surgical technique (Lollipop or Inverted-T?), whether the operation will be combined with breast augmentation (implants) (this is the most significant cost determinant), the surgeon's experience, and the standards of the hospital where the surgery is performed. Precise, personalized pricing can only be determined after a detailed examination with Assoc. Prof. Dr. Yalçın Bayram.
Frequently Asked Questions About Breast Lift Surgery
Can I breastfeed after breast lift surgery?
Breast lift is a technique that preserves milk ducts more compared to breast reduction surgery. In modern techniques, the connection between the nipple and milk ducts is preserved as much as possible. However, as with any surgical intervention, there is a (low) risk that breastfeeding potential may decrease. This risk should be discussed in detail with your doctor before surgery.
Do breast lift scars disappear completely?
No, the scars are permanent but they "heal" and become less noticeable over time. The final appearance of the scars will be lighter than your skin tone, and their prominence will decrease. Assoc. Prof. Dr. Yalçın Bayram plans the incisions in the best possible locations for concealment and will recommend specific scar treatment protocols (creams, silicone sheets, etc.).
Are the results of breast lift surgery permanent?
The results of the surgery are long-lasting. The operation turns back the clock but does not stop it. Gravity, aging, future pregnancies, or significant weight changes can cause some degree of re-sagging over time. However, your breasts will never return to their pre-surgical state.
Will there be sensation loss after breast lift surgery?
Temporary loss of sensation or numbness in the nipple and breast skin after surgery is very common. As the nerves heal, this sensation usually returns significantly within 6 months to 1 year. The risk of permanent and complete sensation loss is very low.
Is placing only an implant sufficient for sagging breasts instead of a breast lift?
No. This is one of the most common misconceptions among patients. If there is sagging (ptosis) in the breast, placing only an implant will not correct the sagging; on the contrary, the weight of the implant can pull the breast down further, causing an artificial appearance known as a "snoopy deformity" (sagging breast with fullness only on top). A sagging breast must be lifted (skin envelope tightened) concurrently with implant placement.
